doc_refs: TEMPLATE 01–04, DIRECTORY
type: structural-summaries
note: |
Summarised rather than transcribed at Mo's direction (1 Sep 2026). These are
blank fillable forms — transcribing empty fields adds nothing for grounding
or article generation. Captured here is what each form is FOR, the standards
it implements, and any hard values printed on it (those ARE usable facts).

# Templates 01–04 and the Directory — structural summaries

## Template 01 · Fire Door Inspection Log (3 pages)

**Purpose:** Records both duties the Fire Safety (England) Regulations 2022 place on residential buildings over 11 metres — quarterly checks of fire doors in common parts, and annual best-endeavour checks of flat entrance doors.

**Form structure:** § 01 Inspection details (Building / Inspection type — a tick between *Quarterly, common parts* and *Annual, flat entrance doors* / Date / Inspected by, name and role / Next inspection due). § 02 "What to check on every door" — the guidance block, reproduced below. Pages 2–3 are the per-door record grid.

**The eight checks printed on the form (usable content):**
- Door closes fully and engages the latch from any open position, under its closer alone.
- Self-closer fitted, undamaged and correctly adjusted.
- Intumescent strips and smoke seals present, continuous and undamaged.
- Gaps consistent and typically around [3mm, no more than 4mm] at head and sides; threshold gap appropriate to the specification.
- Glazing intact, fire-rated, and beading sound.
- No unauthorised alterations — no planed edges, missing hardware, added ventilation grilles or non-rated locks.
- Signage present — fire door keep shut, or keep locked, as applicable.
- Door not wedged, propped or obstructed — and hold-open devices, where fitted, release on alarm.

> Photograph every defect and log it into your maintenance system with a reference. Record that reference in the door record overleaf so the two can be reconciled later.

## Template 02 · Legionella Monthly Monitoring Log (3 pages)

**Purpose:** Monthly sentinel-outlet temperature monitoring — the backbone of routine legionella control under ACOP L8 and HSG274. Records must be kept for five years.

**Form structure:** § 01 Monitoring details (Building / Month and year / Monitored by, name and role / Risk assessment reference and date / Responsible person). § 02 The targets. Pages 2–3 are the outlet-by-outlet reading grid.

**The three target values printed on the form (usable content):**
| Measure | Target | Condition |
|---|---|---|
| Stored hot water | 60°C+ | At the calorifier or water heater. |
| Hot at outlet | 50°C+ | Reached within one minute of running. |
| Cold at outlet | Under 20°C | After running for up to two minutes. |

> Record the reading you measured, not the reading you expected. Give every failure an action reference and carry it through to a documented close-out.

## Template 03 · Incident Report (1 page)

**Purpose:** A single-page structured incident record for building and security teams.

**Form structure — four numbered blocks, all blank fields:** § 01 Incident summary · § 02 Account · § 03 Actions and evidence · § 04 Escalation and follow-up.

*No printed guidance content beyond the section headings — the value is the structure, not the copy.*

## Template 04 · Emergency Contacts and Escalation (2 pages)

**Purpose:** The one sheet a night team or concierge needs at 3am — who to call, in what order.

**Form structure — four blocks:** § 01 Building details · § 02 Escalation ladder (four numbered tiers) · § 03 Contractor call-out · § 04 Utilities and authorities.

**Hard values printed on the form (usable content):**
- **0800 111 999** — the National Gas Emergency Service number.
- **101** — the police non-emergency number.

## Directory · Official Sources (2 pages)

**Purpose:** A one-page-per-side index of the primary UK sources the whole library cites, grouped in four themes. Every entry is an official body or legislation source — no commercial references.

**01 Counter-terrorism and Martyn's Law:** GOV.UK Martyn's Law collection · ProtectUK · NPSA · ACT Awareness e-learning

**02 Security regulation:** SIA Register of Licence Holders · SIA on GOV.UK

**03 Fire and building safety:** Fire Safety (England) Regulations 2022 guidance · Fire safety in the workplace · Building Safety Regulator · London Fire Brigade guidance

**04 Data protection, health and safety, employment:** ICO — video surveillance · HSE — Legionella · HSE — LOLER · HSE — Management Standards · HSE — CDM 2015 · Living Wage Foundation · OZEV — EV fire safety in covered car parks · legislation.gov.uk

*Note: this is the only one of the 26 documents whose text extracts from the PDF, because it is set as live text rather than flattened to an image.*
